Registration formulars

To register for the Conference HS'15, fill out the Registration form and click submit button. This will enter you into the conference's registration database.

If you need also Slovak visa, for yourself or for accompanying person, please fill out Visa form.

Your registration is valid after the successful payment of the Conference fee. For more detail see Conference fee page.
